Premium Book Change Form for 2025 DCF

Complete ONE form per Department. You may add multiple change requests to one form for the same department.

If you need more space in any category, do not complete the form below. Instead, complete the Premium Book Change Form PDF and email to info@danecountyfair.com.

Premium Book Changes Procedure
• All premium book changes are due by November 1. Changes must be submitted in writing to the Dane County Fair office via form, email (info@danecountyfair.com), or mail (PO Box 287, Oregon, Wisconsin 53575).
• Anyone may propose changes – not just department chairs or superintendents. All changes not proposed by department chairs will be presented to the Chair for their recommendation.
• The proposed changes along with the department chair’s recommendation, either for or against, are presented to the Dane County Fair Education Committee and then the Dane County Fair Association Board of Directors for final action at their January meeting.
• The premium book will be uploaded to the Dane County Fair website (danecountyfair.com) by March.
• Judge recommendations will be reviewed by the Department Chair and Dane County Fair Management. Final selection is made by Dane County Fair Management.

Capital Expenditures
• Capital expenditure requests must be submitted in writing with details by September 1 to the Dane County Fair office via email (info@danecountyfair.com) or mail (PO Box 287, Oregon, Wisconsin 53575).
• Requests are presented to the Dane County Fair Association Executive Committee for final approval.
